Next Steps

These are the next steps you will need to complete before you can get started on campus:

** Please do them in this order.

1.  TB Screening: (click here and print)

Schedule an appointment with your medical provider or local clinic. You will need to take the TB screening form with you and have the Physician or RN complete it. Tip: when making an appointment, ask to schedule an appointment with a registered nurse. They’re also able to complete the TB process for you. Please email me a scanned copy or picture for our records to our Kids Hope cell phone (323) 313-4103.

*Note: This is a 2 day process, 48 hours apart.

2.  (LIVE SCAN) - Fingerprinting:  (click here - fill it out - print)

Everyone will need to be fingerprinted. You will need to complete this fillable form, print and take it with you to Super Copy at 2256 Colorado Blvd. #103, Eagle Rock, CA 90041. Hours: 9am - 4pm and they are closed on weekends -This is the location we are approved to use with our new protocols.  I need to let them know in advance that you're going, so please let me know before you go, so they're expecting you.  When you arrive, tell them you're a volunteer for the Kids Hope at Christian Assembly. You won't have to pay, they'll send us an invoice.

3.  Covid-19 Vaccination Record:  

Please email or text me a picture of your Covid-19 vaccination card to our Kids Hope cell phone for our records.

4.  Volunteer Commitment: (click here - fill it out - print)

Everyone must sign LAUSD’s Volunteer Commitment form. It’s a fillable form - open it, fill it out, print it and email or send me a picture of it.
